Abstract

Introduction: current literature recommends Trans-Bronchial Cryo-Biopsy (TBCB) sampling from right middle lobe (RML) or right lower lobe (RLL) in patients with interstitial lung disease (ILD)

Aim and objective: we aimed to evaluate safety and diagnostic yield of TBCB from RUL.

Method: A retrospective single center study. All patients referred to the Meir Medical Center, Pulmonary department for TBCB with suspected ILD were included.  TBCB was performed under general anesthesia using the ?two scope? technique. Procedure details, complication and diagnosis were retrospectively analyzed.

Results: In total, sixty eight patients were analyzes (30 [45%] female and 38 [55%] men), mean age 64 ±11.  The overall diagnostic yield of cryobiopsy was 84% among them however the diagnostic yield from RUL was 77% and in 3% of the cases diagnosis was made exclusively from RUL.

Rate of moderate bleeding was 53%(36) and no severe bleeding was noted. The rate of pneumothorax 5%(3). Both rate are lower then reported in the literature of 78% and 26% respectively.

Conclusion: Diagnostic yield from RUL is lower, especially in idiopathic pulmonary fibrosis. Nevertheless, in patients with suspected HP prior to biopsy, additional TBCB from RUL should be considered in order to increase the diagnostic yield. Larger trails are needed to confirm our result.
