Abstract

Introduction: Chronic respiratory diseases such as asthma, chronic obstructive pulmonary disease (COPD), and bronchiectasis cause a high economic and social burden. However, underdiagnosis and misdiagnosis may lead to patients receiving under or no treatment. Aims and objectives: This study aims to analyze the healthcare utilization in patients with asthma, COPD, or bronchiectasis using nationwide population-based data in Korea. Methods: This study used the National Sample Cohort of the National Health Insurance Service (NHIS-NSC). The NHIS-NSC includes information on healthcare utilization for about 2.2% (1,025,340 participants) of the total Korean population since 2002. Using the NHIS-NSC, we analyzed the healthcare utilization in patients with asthma, COPD, or bronchiectasis between 2004 and 2015 and compared it to those with hypertension or diabetes. Results: Between 2004 and 2015, 71,759 patients with asthma, 5,444 patients with COPD, and 3,737 patients with bronchiectasis were included. The number of healthcare visits was 1.99/person-year for asthma, 3.00/person-year for COPD, and 1.38/person-year for bronchiectasis. The number of hospitalization was 0.03 times/person-year for asthma, 0.25 times/person-year for COPD, and 0.07 times/person-year for bronchiectasis. The number of healthcare visits for asthma, COPD, or bronchiectasis was lower than those for hypertension (8.28/person-year) or diabetes (9.37/person-year). Conclusions: Patients with chronic respiratory diseases such as asthma, COPD, or bronchiectasis showed low healthcare utilization. The number of healthcare visits for chronic respiratory diseases was lower than those for other chronic illnesses such as hypertension or diabetes.
