Panel discussion on Non-CF bronchiectasis in children: Improving patient care - 20 NOVEMBER, 2023

20 November, 2023 | Online18:00-19:00 CETChairs: Dr. Carolina Arez Constant (Lisboa, Portugal), Dr. Ahmad Kantar (Bergamo, Italy)Speakers: Ms. Zena Powell (Aylesbury, United Kingdom), Prof. Andrew Bush (London, United Kingdom), Prof. James D. Chalmers (Dundee, United Kingdom)   Fees: Free for ERS members / €10 for non-membersFind out more about becoming a member   Overview   Non-CF Bronchiectasis is a clinical syndrome of recurrent or persistent wet/productive cough, lower airway infection and/or inflammation, and abnormal bronchial dilatation on chest computed tomography scans. It is a heterogenous chronic pulmonary disorder with many risk factors and aetiologies.   Previously considered irreversible, current data shows that paediatric non-CF bronchiectasis might be reversible over time. Achieving this goal requires early disease recognition leading to appropriate investigations to both diagnose and manage bronchiectasis. Evaluation for treatable underlying causes and effective management are therefore important.   Children/adolescents also require developmentally appropriate care, support and supervision from their parents. Those with untreated non-CF bronchiectasis symptoms from childhood have worse disease and a poorer prognosis. Non-CF Bronchiectasis is still one of the most neglected lung disorders, with high individual disease burden, economic cost and poor quality of life for children/adolescents and their parents. Also, there are large disparities in the standards of care and outcomes between bronchiectasis and other chronic lung diseases.   This webinar will provide an update on current literature on non-CF bronchiectasis and the needs for early disease recognition, prevention and transition from paediatric to adult care.   Moreover, the webinar will offer an opportunity to examine patients experiences, attitudes, preferences, needs, and expectations in patients non-CF bronchiectasis. Patients' perspectives and health goals are at the heart of the care process.   These arguments are among the objectives of the ERS-CRC - Child-BEAR-net (Children's Bronchiectasis Education Advocacy and Research Network). Educational aims To analysis patient experiences, needs and perspectives and the method of patient involvement at different phases of disease managementTo highlight the risks of delayed diagnosis of the underlying causes of non-CF bronchiectasisTo give indications how to improve care for children with non-CF bronchiectasisTo discuss the transition from paediatric to adult care Topics Living with bronchiectasis: a patient perspective - Zena PowellUndiagnosed bronchiectasis in children - Andrew BushOutgrowing pediatric care in bronchiectasis - Stefano Aliberti Target audience: Paediatric pulmonologistsAdult pulmonologistsPaediatriciansInfectious disease specialists Format The webinar will include a patient testimonial, lectures, interactive discussions with participants and two case presentations. Learning outcomes After following this webinar, participants should be able to: Appreciate patients experiences, needs and expectationsRecognise diagnostic errors and delay in diagnosisApprehend how to care for children with non-CF bronchiectasisLaunch developmentally appropriate care and transition from paediatric to adult care CME credit An application for accreditation of this webinar has been made to the European Board for Accreditation in Pneumology (EBAP) for 1 CME credit per 1-hour attendance. If accredited, the CME credit will be granted upon attendance of at least 60 minutes during the live webinar only. What is a webinar? A webinar closely simulates a lecture-based teaching experience. The speaker can interact with the audience, just as in a classroom setting. During the webinar, you will be asked to share your opinion on issues related to the topic using interactive polls. All participants will be able to hear the lecturer and see the slides throughout the presentation. As a participant you will be able to pose questions or discuss ideas with the other participants via the text chat facility and the speaker will respond to the questions via the microphone. Login guidelines More information will be communicated in due course. Please log in to the webinar 20 minutes before it is scheduled to commence. Coffee Talk: Bronchiectasis in Europe

In the latest episode of Coffee Talk, Prof. Stuart Elborn interviews Prof. Stefano Aliberti and Dr Eva Polverino about the largest ever prospective study of adults with bronchiectasis from 28 countries - the EMBARC study. EMBARC is a European Respiratory Society (ERS) Clinical Research Collaboration. Funded by ERS and the European Union Innovative Medicines Initiative, EMBARC enrolled 16,963 patients across Europe to study the causes, microbiology, severity and outcomes of bronchiectasis across Europe. Bronchiectasis is a growing clinical problem and this new data helps us to understand the best way to investigate and manage bronchiectasis, as well as stimulating new research questions and initiatives to improve quality of care around Europe. ERS statement for defining respiratory exacerbations in children and adolescents with bronchiectasis for clinical trials

European Respiratory Journal 2022 60: 2200300; DOI: 10.1183/13993003.00300-2022 Abstract Bronchiectasis is being diagnosed increasingly in children and adolescents. Recurrent respiratory exacerbations are common in children and adolescents with this chronic pulmonary disorder. Respiratory exacerbations are associated with an impaired quality of life, poorer long-term clinical outcomes, and substantial costs to the family and health systems. The 2021 European Respiratory Society (ERS) clinical practice guideline for the management of children and adolescents with bronchiectasis provided a definition of acute respiratory exacerbations for clinical use but to date there is no comparable universal definition for clinical research. Given the importance of exacerbations in the field, this ERS Task Force sought to obtain robust definitions of respiratory exacerbations for clinical research. The panel was a multidisciplinary team of specialists in paediatric and adult respiratory medicine, infectious disease, physiotherapy, primary care, nursing, radiology, methodology, patient advocacy, and parents of children and adolescents with bronchiectasis. We used a standardised process that included a systematic literature review, parent survey, and a Delphi approach involving 299 physicians (54 countries) caring for children and adolescents with bronchiectasis. Consensus was obtained for all four statements drafted by the panel as the disagreement rate was very low (range 3.6-7.2%). The panel unanimously endorsed the four consensus definitions for 1a) non-severe exacerbation and 1b) severe exacerbation as an outcome measure, 2) non-severe exacerbation for studies initiating treatment, and 3) resolution of a non-severe exacerbation for clinical trials involving children and adolescents with bronchiectasis. This ERS Task Force proposes using these internationally derived, consensus-based definitions of respiratory exacerbations for future clinical paediatric bronchiectasis research.

ERS Statement on airway clearance techniques in adults with bronchiectasis

European Respiratory Journal 2023 62: 2202053; DOI: 10.1183/13993003.02053-2022AbstractAirway clearance techniques (ACTs) are part of the main management strategy for patients with bronchiectasis. Despite being a priority for patients, accessibility, implementation and reporting of ACTs are variable in clinical settings and research studies. This European Respiratory Society statement summarises current knowledge about ACTs in adults with bronchiectasis and makes recommendations to improve the future evidence base. A task force of 14 experts and two patient representatives (10 countries) determined the scope of this statement through consensus and defined six questions. The questions were answered based on systematic searches of the literature. The statement provides a comprehensive review of the physiological rationale for ACTs in adults with bronchiectasis, and the mechanisms of action along with the advantages and disadvantages of each ACT. Evidence on ACTs in clinical practice indicates that the most frequently used techniques are active cycle of breathing techniques, positive expiratory pressure devices and gravity-assisted drainage, although there is limited evidence on the type of ACTs used in specific countries. A review of 30 randomised trials for the effectiveness of ACTs shows that these interventions increase sputum clearance during or after treatment, reduce the impact of cough and the risk of exacerbations, and improve health-related quality of life. Furthermore, strategies for reducing the risk of bias in future studies are proposed. Finally, an exploration of patients' perceptions, barriers and enablers related to this treatment is also included to facilitate implementation and adherence to ACTs.
