To highlight some of the emerging themes and controversies regarding the early treatment of interstitial lung diseases; to discuss the common clinical dilemmas and misconceptions that occur in clinical practise regarding the influence of age and comorbidities on the decision to start anti-inflammatory or antifibrotic treatment; to explore the role of risk stratification and early treatment in connective tissue diseases.
