Single maintenance and reliever therapy (SMART) has recently been extended to children aged 6 years and older and is recommended by various therapy guidelines for children at GINA levels 3 and 4. Although the SMART approach has great potential because it is easier for parents and patients and there are data showing it has less potential to retard growth, there are few data from controlled studies to support this approach in younger children. To discuss the advantages and disadvantages of the single maintenance and reliever therapy (SMART) regimen in children; to explain the limitations; to describe which children the SMART approach may be suitable for.
