To discuss the evidence base for accelerated pathways in suspected malignant pleural effusion, including direct to biopsy and criteria for more comprehensive sampling by thoracoscopy; best practice in inpatient pleurodesis, including agents, techniques, timing and detection of the non-expansile lung; the optimal use of indwelling pleural catheters, including drainage frequency, adjuvants and management of complications; and the pathophysiology of symptoms and fluid production and how these might be manipulated to improve outcomes.
