to summarise highlights from longitudinal cohort studies that provide insight into the structural and immunological features of early disease manifestations in chronic obstructive pulmonary disease (COPD); to provide insights into potential mechanisms implicated in early disease, how these manifest in terms of clinical presentation and some potential avenues for early screening and intervention.
