Aims: to provide an expert overview of the importance of non-specific restrictive lung function impairment (small lungs) and how, until recently, this has been ignored epidemiologically and often confused with chronic obstructive pulmonary disease (airflow obstruction); to highlight what we have learned about small lungs, especially from research in low-income countries; to describe the evidence for the developmental origins of small lungs, the predominant association of restrictive spirometry with cardio-metabolic abnormalities, poor prognosis and low survival, and the clinical implications for respiratory medicine and general practice, and particularly for practice in low-income settings; to conclude each talk with take-home messages.
