To provide an expert overview of the current state of knowledge concerning patterns of lung function and cardiovascular and other comorbidities; to highlight that evidence is stronger for restrictive than obstructive defects; to clarify that chronic obstructive pulmonary disease (COPD)-comorbidity links are largely explained by known risk factors, whereas the restrictive lung function-comorbidity link has not been fully explored or explained; to present evidence from low- and middle-income countries linking a low forced vital capacity (small lungs) to cardiometabolic comorbidities; to demonstrate that part of the association between a restrictive defect (lower forced vital capacity) and cardiovascular comorbidity is explained by shared developmental signalling pathways; to describe the clinical and healthcare implications of comorbidities associated with COPD and impaired lung function; to describe how artificial intelligence techniques have contributed to lung function trajectory modelling; to provide take-home messages; to provide a deeper understanding of respiratory comorbidities and their clinical implications.
